Educational Field Trip Driver Form
Anyone driving on a district school event must complete an educational field trip driver form even if they are not transporting students. This form only needs to be submitted once during the school year and updated with new insurance information when coverage expires. Note: A copy of your auto insurance policy declaration page/coverage must be submitted with Driver Form.

Student Accident & Sickness Insurance Program
For families that wish to apply for insurance. Mail completed application to insurance company (see application for mailing address). Do not return it to your child's school office.Note: The District does not assume responsibility for medical treatment (ambulance transport, surgery, hospitalization, etc.) due to accidents.
